I use 3 different profile forms for my characters: one for major characters which is the most complete, on for minor characters, and one for walk-on characters that I want to be interesting without taking over the story. Shown below is the longer of the 3 profiles, but all 3 are available in the zip. The complete profile is the "full" form, minor characters are noted as "half", and the walk-on profile is listed as "walkon".

One note about the profile: I've found that sometimes the table cell sizes (in the physical section) have to be adjust to fit the information I want it to. As long as the total of the cell numbers doesn't exceed 650, minor tweaking works.
